The first cohort: a working partnership
A partner case-management agency referred the first three veterans placed through CHV’s transitional housing model. Their case management plus our model produced durable placements at the 12-month mark. That is the operational proof point new partners get to build on.
Two business days
Partner inquiries get a real response inside two business days. Not a ticket number. Not a generic auto-reply.
Plain language
Our partner Memorandum of Understanding (MOU) is written in plain language. Roles, responsibilities, expectations, exit terms. No surprises.
